Pharmacy

Medical Center Hospital and its pharmacy department operates on core values of integrity, accountability, respect, excellence, and patient-centered care. As a resident and practicing clinician in this institution, these values will be the foundation of all expectations during the PGY1 Pharmacy Practice Residency program as well as the PGY2 Emergency Medicine Pharmacy Residency.

Medical Center Hospital initiated its PGY1 Pharmacy Practice Residency program with two residents in July 2014. Our re-accreditation visit from the American Society of Health-System Pharmacists for this 12-month residency program was completed in 2020, and we are accredited until 2024. In 2018, our PGY2 Emergency Medicine Pharmacy Residency program was started and was officially accredited from 2020 until 2028.

Clinical Services

Decentralized pharmacists provide clinical pharmacy services, including:

  • Daily inter-professional rounding with Internal Medicine, Family Medicine, Critical Care, Trauma, and NICU teams
  • Antimicrobial stewardship
  • Criteria-monitored antibiotics
  • Daily review of appropriate dose and indication of broad-spectrum antibiotics
  • Anticoagulation services
  • Consult service for dosing of warfarin and IV direct thrombin inhibitors
  • Warfarin and DOAC patient education
  • Daily monitoring for safe use of target-specific systemic anticoagulants
  • Pharmacokinetic Services
  • Dosing and daily monitoring of vancomycin and aminoglycosides
  • Dose adjustments for renal impairment
  • IV to PO program
  • Patient education
  • Code blue and rapid response participation
  • Transition of care services (discharge counseling, discharge medication reviews, OPAT, and so on)
